Ohio Immunization Summer for School Attendance 2025-2026

Meningococcal ACWY:  (Grade 12) Two doses of meningococcal (serogroup A, C, W, and Y) vaccine. Second dose on or after age 16 years. If the first dose was given on or after the 16th birthday, only one dose is required.

Shot records are required to be submitted to the school by October 15, 2025 showing the vaccination requirements for school attendance have been met 

OR 

You must sign a waiver stating that you are electing to not have the 2nd dose and understand that if there is an outbreak at school, your child will not be allowed to attend until the outbreak is resolved.  (Exemption Waiver Attached)

Schools are required, by law, to report any students who are out of compliance with regards to this vaccination to the state as of October 15, 2025.  If your student has not met the vaccination requirements NOR signed a waiver, they will not be allowed to attend school until they are in compliance.  

This vaccination is available at the Health Dept. for little to no cost or you may contact your child's physician to arrange for your child to receive the vaccination.
